As a health professional, I keep records of my patients
(and therefore their identifying details).
The use and storage of these
records are subject to several sets of legal and professional
requirements; all intended for the patient's best interests
I keep these records electronically. Any paper information you provide
me with is scanned into electronic format before being securely shredded
Patient data will not be shared with anyone without patient consent
I will undertake all reasonable actions to protect patient data
I maintain contact with patients by post, email, telephone and text
messaging
Patient records are kept for at least 7 years following
the last contact. For minors this is at least 7 years after reaching the
age of majority (18) i.e. age 25
Website Logs
Like most standard website servers we use log files. This includes website
visitors internet protocol (IP) addresses, browser type, internet service
provider (ISP), referring/exit pages, platform type, date/time stamp, and
number of clicks to analyse trends, administer the site, track user's
movement, and gather broad demographic information. IP addresses, etc. are
not linked to personally identifiable information
